- Postal address, telephone number, email address and any other contact details you provide.

3. Purposes of processing
We process your personal data for the following primary purposes, which are necessary for the clinical relationship and for delivering our services:
- Providing medical and psychological services, and carrying out clinical assessments, diagnoses and follow-up of the therapeutic process.
- Compiling and safeguarding your clinical record and care history.
- Booking, confirming, rescheduling and following up on your appointments.
- Contacting you about matters relating to your care and responding to your requests.
- Meeting the legal, health and administrative obligations that apply to us.
We may also process your data for the following secondary purposes, which are not essential to the delivery of the service:

- Assessing quality and improving our services through surveys.
If you do not want your data processed for these secondary purposes, you can object by writing to direccion@centroidenta.com. Objecting will not be grounds for refusing you the services you request, and it will not affect the clinical relationship.
4. Processing of sensitive personal data
Processing your sensitive personal data relating to health requires your express consent. By beginning and continuing the process of care, and by providing that information, you consent to it being processed for the medical and psychological purposes described in this notice.
This data is processed solely for the purposes of clinical care and is protected by reinforced security measures, whether administrative, technical or physical, designed to prevent its loss, misuse, unauthorized access, alteration or disclosure. Access to your clinical information is limited to the staff directly involved in your care.
5. Transfers of personal data
Centro IDENTA will not transfer your personal data to third parties without your consent, except in the cases set out in article 37 of the LFPDPPP. These include transfers required by law or by treaty, transfers necessary for medical care, and transfers requested by competent authorities acting within their powers.
Where information is processed by suppliers who provide services to us, such as scheduling or data storage platforms, that processing is carried out on behalf of and in the name of Centro IDENTA, under the figure of the data processor. It does not constitute a transfer requiring your further consent.
6. How to exercise your ARCO rights and withdraw consent
You have the right to know what personal data we hold about you, what we use it for and the conditions of that use (Access); to ask us to correct your information when it is out of date, inaccurate or incomplete (Rectification); to have it removed from our records when you consider it is not being used in line with the applicable principles and duties (Cancellation); and to object to your data being used for specific purposes (Objection). Together these are known as ARCO rights. You may also withdraw the consent you have given us to process your data.
To exercise any of these rights or to withdraw your consent, send a request to direccion@centroidenta.com including at least:
- Your full name and a means of sending you our response.
- Documents proving your identity or, where applicable, the legal representation of whoever submits the request.
- A clear description of the personal data the request concerns and of the action you are asking for.
We will respond within the periods set out in the LFPDPPP. Please note that exercising these rights or withdrawing consent may make it impossible to continue providing medical and psychological services, and that certain clinical data must be retained for as long as the applicable health regulations require.
7. How to limit the use or disclosure of your data
You can limit the use or disclosure of your personal data by sending your request to direccion@centroidenta.com. Where the request is upheld, it will be recorded so that your data is not processed for non-essential purposes, such as sending promotional or informational communications.
