What chronic tension-type headache is
Chronic tension-type headache is a headache usually described as a band of pressure or tightness, mild to moderate in intensity, present on most days over a period of months.
Unlike migraine, it is not usually throbbing and does not tend to be suddenly disabling. What wears people down is how constant it is, and the effect that has on mood, sleep and concentration.
Why the tests come back “normal”
Tests are usually normal because chronic tension-type headache does not come from a lesion. It comes from a combination of pericranial muscle tension, sustained stress and sensitization of pain processing.
Stress and posture keep the muscles of the neck and scalp active, and a nervous system on alert amplifies that signal. It is a cycle, and cycles can be broken.

- Near-daily pain, mild to moderate
- Tension in the neck, shoulders and scalp
- Worsening with stress and as the day goes on

Frequently asked questions
Is this the same as migraine?
No. Tension-type headache is usually a bilateral pressure or tightness rather than a throbbing pain, and it rarely comes with intense nausea. Treatment shares the nervous system regulation work, but the mechanisms differ.
Can stress really cause a daily headache?
Yes. Sustained stress keeps muscle tension and nervous system alertness high, and that amplifies pain. Working on this regulation is central to breaking the cycle.
Is taking painkillers every day a good idea?
Frequent painkiller use can make headache chronic, in what is known as medication overuse headache. Part of the work is bringing that use into order safely and under medical supervision.
