What chronic fatigue syndrome is

Chronic fatigue syndrome, also known as myalgic encephalomyelitis, is intense and persistent exhaustion that is out of proportion to any effort made, lasts for months and does not improve significantly with rest.

One characteristic feature is post-exertional malaise: after physical or mental activity that used to be manageable, symptoms get markedly worse, sometimes hours or days later.

Why the tests come back “normal”

As in other central sensitivity conditions, standard tests are usually normal because there is no single lesion that explains it. What is involved are changes in autonomic regulation, in sleep, in how exertion is processed, and in the stress axis, the PNIE system.

That does not make it any less real. It means the problem lies in how several systems are regulated rather than in a damaged organ, and that treatment has to be careful not to overload the system.

Frequently asked questions

Is this the same as being tired?

No. It is disproportionate, persistent exhaustion with post-exertional malaise, and it is not resolved by sleeping more. It has a serious effect on daily function and calls for a specific approach.

Will exercise help me or harm me?

It depends on how it is dosed. Poorly graded exertion can trigger post-exertional malaise. That is why we work on activity very carefully and individually, rather than through generic programs built around doing more exercise.

Is it a psychological problem?

It is a real biological condition in which stress regulation, sleep and the autonomic system all play a part. The psychological side accompanies and modulates it, but that does not mean the fatigue is invented.
