Central sensitization
The mechanism that explains why pain and other symptoms are amplified and persist when tests show no lesion.
What central sensitization is
Central sensitization is not a specific illness but a mechanism: an amplification of how pain and bodily signals are processed within the central nervous system. It is the common thread running through many persistent symptoms.
When this mechanism is active, the volume on the body’s alarm system goes up. Stimuli that would not normally hurt are experienced as painful, and the discomfort spreads and stays.
Why the tests come back “normal”
Imaging and laboratory tests look for structural damage or inflammation. Central sensitization is a change in how the nervous system functions, not in the visible structure of tissue, which is why tests usually come back normal.
Understanding this mechanism changes the whole approach. The goal stops being the search for a lesion that is not there and becomes the gradual recalibration of a system that has grown too sensitive.
Common symptoms
- Pain that is amplified, spreads or persists with no lesion to account for it
- Hypersensitivity to touch, light, noise or smells
- Fatigue and unrefreshing sleep
- Difficulty concentrating, or brain fog
- Symptoms in several body systems at once
How the IDENTA Model approaches it
At IDENTA the first step is to name the mechanism correctly through an integrated assessment, which in itself usually eases a good deal of the uncertainty.
From there we work on pain neuroscience education, autonomic and sleep regulation, evidence-based psychotherapy and graded movement, all aimed at turning down the volume on the alarm system.
Central sensitization is rarely addressed with a single tool. What works is the coordinated combination within the IDENTA Model.
What you can expect
- To finally understand why your symptoms are amplified and why they persist.
- A plan that recalibrates the system instead of searching for a lesion that is not there.
- Concrete strategies and measurable follow-up.
This information is provided for general education. It does not replace an individual medical assessment and is not a diagnosis. If you live with these symptoms, an initial assessment allows your case to be evaluated as a whole.
Frequently asked questions
Is central sensitization the same as fibromyalgia?
Not exactly. Central sensitization is the mechanism; fibromyalgia is one of the conditions in which that mechanism plays the leading role. It also appears in chronic migraine, irritable bowel syndrome and others.
Can it be reversed?
The nervous system is plastic, so it can be recalibrated. This is not an immediate cure but a gradual reduction in the amplification, through a sustained approach.
Does this mean the pain is not real?
Quite the opposite. The pain is entirely real. What changes is where it comes from, which is how the nervous system processes it rather than any visible damage.
